Kazuhisa Hashimoto added the sequence while porting the shooter Gradius, because he kept dying and needed a quick way to arm himself with power-ups during testing. He meant to remove it before release but forgot — and players discovered it, turning a debugging shortcut into one of gaming's most enduring secrets, reused in dozens of later games.
